It looks like it's made w/a square of paper plus extra for the flap on top. It appears to be made the same way as a box without a lid, then pinch the sides together. KWIM?
Never seen this before, so I'm sorry I don't have a template.
Okay...here is the template. Basically, you are dividing the 8.5 x 11 cardstock into thirds one way and fourths the other. The squares are 2 7/8 each. Score and fold on the dotted lines. (It will look like a tic-tac-toe board!) Then fold the corner squares on the diagonal. If you need more information, just let me know and I'll try to write up better directions.
