I saw a sample of this box in a store in Buffalo last week and just had to have one of my own.
The stamp is from Holly Berry House, and the stamp itself is the template. How cool is that?!
The box topper is also Holly Berry House and is a medallion stamp. Medallion stamps are progressive stamps that are stamped and embossed several times and then cut down to form the decreasing layers then popped up on dimensionals.
I don't know the name of either stamp as they're not mentioned anywhere on the block at all.
I stamped using VersaMark on black and embossed with gold EP then colored with H2Os. Took forever but was worth the time.

The box is Victorian Breeze II in their treasure box catagory. I had to go look at the stamps to see how they did the template for the box. I really like what you did with it!
