Here's a great thread which has templates for all different sizes of match boxes. They are referred to as "nugget boxes" because they were designed to hold different amounts of Hershey Nuggets.

Not sure if you still need one, but here's a free one that I made :
Mel Stampz: Fond Farewell: Mo Valentine's ATC matchbox
It has a liner for the inside box as well. I just love them for adding patterned paper.
If you think you might like to use it & the 4shared site gives you any trouble, please just let me know. I've switched to adobe for sharing templates, so I'm not sure how well 4shared is behaving itself these days. ;)
