Default need help with a gift card holder

It's that time of year again, and I've lost my sample. Does anyone have directions on how to make the card that you pull the ribbon and the gift card comes out the top? I love putting little gift cards in there, makes it a little more fun.

Attached is a page I made for my idea book using a file I downloaded from SCS this spring. I could not find the thread, and decided to attach the file... Credit to StormRose for the file.

it's super simple... just a basic template that you can make as minimal or elaborate as you'd like. I'm making some different ones (same template) this year to stick in stockings...
