Gift Card Holder

I am wondering if anyone has directions with picture links as to how to make a gift card holder. Any suggestions/help would be greatly appreciated.

At Christmas time I did a search in the gallery and came up with these... Gallery at Splitcoaststampers

Many of them include the directions and also, starmaze had a tutorial on her blog that used an envelope to make one which was different and nice.
