Sliding pop up Box Card??

I saw a card that I would love to have a template for. I don't know if it has a specific name, so I will try to describe it well. When you look at the card, it looks like a normal card, but then you slide it down, the bottom of the card becomes a small rectangle box and there are two paper pieces that pop up from the box. It was adorable and I would like to make one...does anyone have a template for this???
