I was taught to do it by punching two holes, pushing the ribbon ends through from front to back (leaving a loop on the front of the cardstock), and then crossing the ends and bringing each one back to the front through the opposite hole.

I was tough to punch four hole making a diamond shape with a 1/8 hand punch. With the two end ribbon put it through the two front hole leaving a loop from the front cs crossing the ribbon from the back coming out from the other two hole and tiring the two end over the loop to a nock and you'll have a perfect bow
