looking for box pattern

I am looking for the pattern for the box on pg. 32 of the catalog. Anyone have any ideas where I can find it?

I am not sure this will help, SU has the pattern for this box in the Portfolio Series "Gifts & Giving" on page 39.

I don't have the portfolio series. Does anyone have a template for this box? PLEASE
