pencil pouch template?

Does anyone have a template for one of these, or can you direct me to a site that has one? I need a small pouch that would hold 2 pencils?

Checked my bookmarked sites but couldn't come up with a pencil pouch per se... Maybe the chopstick holder from Mirkwood would work? Template and directions are here: http://www.ruthannzaroff.com/mirkwoo.../chopstick.htm
