This is a neat card! I like how you layered the paper. It gives texture.
When I think of a porthole, I remember "bulky" brown, wooden circular things (or shiny brass ones) that would go "outside" the circle "window" you've cut here.
Another thought, the hook? If you wanted to, you could use a real fish hook (just layer on top maybe?) and add fishing line to the top of the card... or use wire and shape it to look like a hook.... Oh.. just had a thought. If you suspended the hook from the upper right area (as if the fish had just swam past it, you could put small text in the lower right (possibly computer generated or handwritten) to say "off the hook". That could be a cute thing to send at just he right time, you know? It would also give you the "3" elements to give balance and interest. Just a thought.
