I hope you can view this template - I made it for my OSW and I have been asked for it quite a few times so I though I would try to load it - hope it works. Some measurements may be wrong measure yourself once you print it!

Thank you so much for sharing this with us. I am hoping to be able to see the measurements. I printed it once, but wasn't able to see them. I iwll try again later so I can play with the contrast. This has been a HUGE help! Thanks for going through all the work and putting this up for us!
