These three matchbook-style post-it holders were for my children's teachers as back-to-school "thank you" gifts. I'm just now getting around to uploading the pics. :)
I made the template for these myself. *brag brag* I found the post-it pads at the local 100 yen store (like a dollar store) and then just designed the cover based on the size of the pads. I did a mock-up of them using copier paper first (cheaper that way!).
The cute little pink pencil is at the top under the bow. Isn't it cute?

I don't really have a pattern I could share. These post-its were an odd shape. They were not the normal American-size but something I got here in Japan. So I'm not sure if my template would be of any use to you all in the States. :/ Sorry! If it helps you make your own, you can do a mock-up with cheap paper (like notebook paper or computer paper). Be sure to give it a bit of extra give since cardstock is thicker and stiffer than cheap, thin writing paper. HTH!
