I used the box card pattern found on the web (have seen several examples here too). This is the box shown folded, with the lid on it. I made it before I had SU ink. I have since bought all the spots, so will try again.

Thanks for the comments. I really like this pattern too. I think the box could be adapted to different sizes but I followed the original instructions. They were typed by Amy Celona on a stamping site that I subscribe to.
I started out with a 7.5 inch square. Score in 3rds each directions, i.e. 2.5 and 5" marks on each side.
Trim diagonally from the outside score points, i.e. cut a triangle off on 4 corners.
Fold into the centre on all score lines, pushing the side panels into the centre.
Box Top
Cut a square 4-1/4 by 4-1/4 inches.
Score in from each side 7/8"
Cut the north/south score lines only to the intersection of the east/west score lines.
Fold in the square flaps and glue inside the lid.
