Super easy! Cut your card stock at 41/8 and then score at 3.5, and again at 7. Fold. Use the slit punch on each end. Line up the little nubbin on the side of the punch in the middle between the edge of the card and the edge of the fold. Layer the whole piece on another piece of cardstock.

Okay, color me stupid - I am trying to make this . . . is it 4-2/8 x 11? I scored at 3-1/2, then turned it around and scored at 3-1/2, or did you mean 3-1/2 and 7? Then, when I fold it up, why is the end I'm folding it into too short? Thanks for any help, I love this!

Sorry Kelly... you are right, it's 3.5 then at 7 from one side.... or 3.5 then another 3.5 from the previous score line! Sorry about that... I'm not so great at directions. ;)
