Card box with flap

I made a box to hold a set of eight cards I made for son's teacher. Working at the last minute, I couldn't find the template of one I had seen here before, so I quickly just made one up. Someone pm'd me asking for the template and directions, and I couldn't send her a file in a pm, so here it is for anyone to use. Let me know if the instructions are unclear.

So odd, thought I added it. Perhaps last night's computer crash caused it to disappear. Trying again:
