I found the idea for this in an old "Card Fashion" magazine. Here is the finished product:

The first template is for the white base of the card. Obviously, you could make this out of patterned cardstock and save yourself the hassle of covering it with paper.
My big hint is to score and fold first, then make your diagonal cuts. I didn't fold first, so when I made the cuts, my folds were "backwards" and not pretty!
The second template is for the patterned paper cuts.
For the pullout portion, I made mine 4 inches wide, then decided the height after I glued the wrap portion together.
