I think the box itself is the Kraft Gift Box on page 161 from the regular catalog. Is that what you are asking? #124106. Looks like they are $2.95 a piece.

Here is the supply list. It looks like the flower is made with the White Creped Filter Paper and the Floral Frames Framelits Dies, layered together, and "scrunched" to make the flower.
