Tissue pack holder

Does anyone have a template for a tissue pack holder? I saw one recently that looked like a little purse; you open it up and the tissue pack fit snugly inside with a square punch out for the tissue opening.

Any help would be appreciated - I didn't write down the dimensions when I saw it (shame on me!)
