Altered Rolodex Questions

i have seen the gallery, full of beautiful altered Rolodexes. i would like to make one, but have a few questions.
1) how do you punch out the holes at the bottom of the card so i fits in the holder?
2) where (besides Staples) can i find rolodexes and the cards?

At staples, office depot, office max...someplace like that there is a rolodex punch...actually made by rolodex. You can find the rolodex files at walmart or target if you don't want to get them at your local office supply store. You might also be able to google a rolodex punch and see what comes up.
