CD Cases...HELP!!

I saw a pic in the gallery yesterday and now I can't find it to find out who did it, but what I want to know is... How do you do it? Can some of you wonderful ladies help? I realy love the look of this and would like to do some for my daughter and friends for their Christmas paint cans. Any help would be greatly appreciated!!! Thanks in advance!!!
