Tag Book Instructions

Hello! I am looking for instructions on how to make the tag books I see in the Dirty Dozen Gallery. They are great! ARe there instructions posted anywhere? Thanks for your help!

The tag book I used is from K & Company (Lifes Journey line)

Beate & Lana made their own by making a huge tag, scoring where the hole part of the tag would be (under the angled cuts), then scoring the rest at the halfway point. Does that make any sense? You can see it much easier on Keri's tag book that Lana made:
