Here ya go:
Forums at Splitcoaststampers

Should be in there somewhere . Typing box in the Search here gave me a bunch of options, and I'm lazy! I got the template from Amy Celona's website, so it was a bit easier for me to narrow down the thread by searching for her name!

I did one and here are the instructions from the sheet we used:

Card stock 7 1/2" x 7 1/2" square. Score at 2 1/2" from all 4 edges. Cut off corner edges diagonally. Stamp on cs, Fold sides up and push diagonal cut piece in and crease for fold line. Top: 4 1/2" x 4 1/2" square. Score at 5/8" from all four edges. Fold along score lines. Cut 2 slits on opposite ends up to fold line. Tuck cut tabs in and attach to longer side pieces to form lid.

I remember something not fitting right when we did it, so you might want to play around with it on scratch paper first. I tried to attach the picture here but couldn't, so just email me if you want a picture sent.

HTH
