Default Votive Candle Holder Box Template Needed

Does anyone know where I can find a votive candle holder box template? I want to make up a few to use for door prizes and I've been searching and I can't find it anywhere. I'm new to this site so I'm not real savvy at negotiating around it just yet.

Here's a version using vellum windows for tealight holders. See if you like this....

Here's instructions that I'm sure I copied from this site!! Have fun

Cut a sheet of card stock to 8 1/2" x 8 1/2".
Score at 3", turn; score at 3", turn; score at 3", turn; Score at 3".
Cut out the corner squares, following the score marks so that your large piece of card stock looks like an "X".
Stamp the "innie" side of the scored card stock.*
Turn card stock over and punch each of the four sections with large rectangle punch (also held upside down).
Place vellum card stock over the holes (which you have also stamped).
Punch each top corner with 1/8" punch and tie with hemp. You can do the 1/16" punch if you want to cause frustration with your customers. The hemp varies in thickness so it was just easier for them to use the 1/8" hole.
Place tealight in box!

*Tip: There is a right side of a score and an inside of the score. The indented side of the score is what becomes your spine or outside corner of a card or box. The raised side of the score is what folds on itself or on the hidden side of your card or project. Card stock folds better if you fold on the scores correctly. I tell my customers to stamp the "innie" side of the score. I learned this when working in a print shop.
