Default How do you make a pocket card?

I've been seeing these in the gallery and would like to make one. Is there a template for this? I've tried to find the information by doing a search but I'm not coming up with anything useful. Thanks!

4.25" by 10.5" strip of cardstock. (You can use 11" but I prefer to snip a bit off the end)

Score at 5.5" and 9". Fold in half and then fold the extra bit over.
