We have gallery moderators "manning the gallery" approximately 18 hours a day, and we manually approve uploads, making sure they are in the correct gallery, etc. Sometimes you will see that others go through quickly without approval needed - those are for admin, moderators, dirty dozen members past and present, and also dirty dozen fan club members. All of these members have instant upload status and don't have to wait for their uploads to be approved.
