Default Star card from So Shelli blog?

Hello everyone,

Happy New Years!

I was reading Shelli's blog this evening and came across this beautiful star ornament (that begins by looking like a tag). I was unable to cut and paste, but here is the link to the blog and it's on the second page:

I wanted to know if anyone has attempted to make one, and if they have, if they have a template or more directions? I wasn't able to understand the directions well enough to pull this off.
