Mayflower Template

Does anyone know where I can find a template for the Mayflower pictured on page 31 of the current IBC? My MIL needs it. It may be posted here somewhere already but I'm in a huge hurry right now, the baby is crying and so it's easier for me to just start a new thread. Thanks for understanding!
