Pencil Case Template?

Does anyone have the template or dimensions to which the CS should be cut? Is it just flat or is the bottom like the end of a pillow box?

The Wilsonart countertop samples don't have...

The Wilsonart countertop samples don't have writing on them (a paper sticky label on the back) and have nice rounded corners. No extra work of removing the printing!
