I have seen test tubes with screw on caps at Nasco in Fort Atkinson, WI. Not sure of the price but saw them as recently as last week.

If you don't have a perf tool, you can also use a sewing machine and do a bunch at a time - works well for anything that you want to detatch ("raffle tickets" and "prize slips")
