Have you already made the card? Is it Clarence? How old is the child? How serious is the illness? Depending on these things, you could have the clown hanging from a high wire with a "hang in there" verse. You might have him juggling good wishes/happy thoughts. Actually it is hard to offer an idea without answers to the above ?'s.

What about something like "Better get better...and I'm not clowning around!" You might want to play around with it and make it work for you. You could put the I'm not clowing around part on the front if there was space, I don't know...lots of options.
