Default folded "passport" booklet

I had directions for this booklet from online a long time ago. It got thrown out when cleaning out files. The site was taken down. This booklet appears on the first Tim Holtz dvd which I saw, but don't own. He uses it when demonstrating distressing tage. Does anybody have the directions for this pocket style booklet? Thanks for your help. I can't make these without looking at the directions. You'd think after doing this enough, I'd remember how to fold it.

I couldn't find the directions, but this page has a photo at the bottom that fits your description of a passport book: http://www.milllanestudio.com/Past_classes.html

Maybe that will help identify it, and someone else will know whether or not directions are available online.

That's the one. The frustrating part is that I saw this long before it showed up on Tim's dvd. Was hoping it was available without the dvd. Thanks for finding it. She said the workshop was two hours long, but making the booklet takes a very short period of time. It's the tags and such that increase the time. Hope someone here can see it and post directions. Thanks for the link.
