Matchbook card template

Hi..anyone have the template for the matchbook card..i think that is the name of it. it folds over and tucks in to look like a matchbook.

I don't know if I did it the "right" way, but I just added an extra 1" on the bottom of my standard card & scored it (at 1" from the bottom) to fold up.
