I got a cotton white opaque Stazon from Addicted to Rubber Stamps - the kind that comes with the blank pad and the refill which you have to ink yourself. I followed the instructions (and then kept adding more ink to the pad after their "teaspoon" didn't seem enough) but I just can't get enough ink on my stamps and it won't make anything but a ghost of a stamp image (real watered down looking) on paper. Did I not shake the refill bottle enough? Could it be defective? Help!?

The white StazOn is meant to be used on non-porous surfaces such as acetate, metal, glass, etc. It won't work well on paper at all. You'd need the white craft pad for that.
mine did not come with instructions but I know I used more then a teaspoon on the pad. I inked it up like I ink the other pads I have, till it looks wet

What are you stamping on, if I may ask? Like Jen said, Staz-on is not meant for paper really, but plastic, metal, glass, etc. It'll just soak in to the cardstock.
I imagine also that the inker really needs a goooood shaking up before applying, too.
