I didn't make the box, but I saved instructions from one I saw posted by Barb Mullikin for the same thing. Here are her dimensions.

Outside cover:
4 x 5 US White
score at 3/4, 2 1/4, 3, 4 1/2

Box bottom:
2 15/16 x 5 7/16 scored at 3/4 all around

Hers held 4 nuggets, if I remember right.
