There was one done in the Dec/Jan issue of Papercrafts Mag if you get that. It is on page 26. It is not done with stamps but I just modified it.

Here's one in my gallery
Gallery at Splitcoaststampers
It's 5 x 8 inches and folded on the 3 and 6 inch lines.
When using the slit punch in the middle section put it all the way in.
Pretty easy

This is an actual card but it has a pocket to hold a gift card. It was made by SHELLI GARDNER from Stampin' Up (just in case you didn't know who she is!!) At the bottom of the page is where you "click" to get the template for the vellum pocket. I've made this with Pretty Princess & there were really cute!!!
