I had to think about it for a while myself! This is what I ended up saying.

"Dear __________,

My name is Dianne Bolden, and my hobby is making greeting cards. I spoke with your activities director and asked who at Fountain West might enjoy receiving cards, and he gave me your name. If it's alright with you I'd like to send you a card every once in a while.

With kind thoughts,

Dianne Bolden"

While my intention is to send one every month I didn't want to promise that in writing, and I wanted them to know where I got their name. I didn't want some poor soul wondering if she was getting alzheimer's because she couldn't remember who this person is who sent her a card, or to freak anyone out wondering where some stranger got their name.
