This is a PDF of instructions for a 4 2/8" box and a matching envelope template that fits it.

(The box actually measures 4.5 inches square & 1 inch deep.)

No printable template for this one, due to its size, but hopefully the instructions are clear. If not, lemme know. :O) It's really easy one you try it once.
