I finally made one to add to this gift set I uploaded in August: Gallery at Splitcoaststampers which I finally gave to my SIL for her birthday on December 31.
My modifications: Matboard size is 8 1/2 x 4 3/4, solid cardstock for the inside covers is 8 1/4 x 4 1/2. Months are printed on six sheets of parchment paper (two months per page), pages folded in half and the bottom trimmed off to 8 1/4 tall. I ran double-sided scotch tape along the open side for reinforcement before punching. Three clasp rings are used instead of two and I used my crop-a-dile to punch the matboard. The double-sided month tabs are computer printed on Mellow Moss, punched out with the Designer Label punch, folded in half and the sides trimmed off. For the front cover and post-it note holder, the words are computer generated on Blush Blossom, and overstamped with the frame image using versamark and ruby EP (love that stampa-ma-jig!). The holders for the matching pen are made with two strips of cardstock 3/8 x 2 inches, threaded through holes made with the slot punch.
