I've wanted to make this ever since I saw Karen Thomas make a mini post-it pad cover with pencil holder closure on Carol Duvall (episode CDS-102. I combined hers with Marcia Howell's pattern: Forums at Splitcoaststampers This was a challenge, but I have a thing about keeping the matching pen with the notepad! Plus, I like that the pen "holders" serve as the closure for the notepad holder too. The Pentel TKO pen is just a slimmer version of the RSVP pen.

As far as a pattern, Tammy, I just adapted Marcia Howell's directions: Forums at Splitcoaststampers Basically, Marcia's paper sizes are adjusted for the bigger post-it pad and I just added the pen "holders," (ala Karen Thomas's HGTV version) which are two strips of cardstock (3 1/2 x 1) scored at 1/4 inch along each of the long sides, then folded to meet in the middle and then glued with a glue stick for strength. I used Terrifically Tacky Tape to attach them to the cover, one at the bottom of the front and the other at the top of the back (see photo's), so when the pen is inserted it keeps the holder closed.
