This was fun! I didn't actually fold my shirt. I created it by using the following pieces of double sided paper.
3 x 3.5 rectangle for body
2 1 3/8 square punches for sleeves
1/2 x 3 for collar-I just eyeballed the folds to create the collar.
I split the neckline with my paper snips...my dad doesn't wear ties ;)

I have seen lot's of them shirt/tie cards on here.
I have never made one before. I just might have to try this one out. Thanks for tellin us how you made it.
