I just leftover pieces to make my matchbooks. Here's how I did it.
Really you just need to measure your post it note and add a bit (1/2 inch) on each side and make the front flap 1/2 inch (this is the part you staple) and the top spine 1/4 inch.
I got my post its at the dollar store and I split the pads in half (to save money).
My post its were 1 1/2 x 2 inches. So I made the paper 2 1/2 inches wide by 5 inches long. I scored at the 1/2 inch mark, then again after 2 1/4 inches (so at the 2 3/4 inch mark), then again at 1/4 (so at the 3 inch mark) and that was it. I stapled the 1/2 inch flap and added my post it with a glue dot.
Now you can adjust the measurements for whatever post it you find.
