although this is a OSW I actually split the one sheet into 4 quarters and did them in different cardstocks. If that doesnt make sense (because my brains are fried today) PM me...

Sorry ladies, I can't upload the template because I did it in Visio (cadcam softare) I have put it in word but as my scanner is dead I can't actully upload the template as its a word document. I would happily share if technology wasnt getting in my way - if you would like this template PM me with your e-mail and I will e-mail it to you
