I used patterned paper by Paper Salon to create the little gift bag. For the pattern, I just took apart a little cardstock bag from the Container Store. They are sold flat and you sort of fold them up to form the bag... very easy to just unglue the edge, unfold, lay out over a piece of paper and just trace. Mark the fold lines and score with a bone folder or scoring blade on your paper trimmer. I punched the little handle hole at the top with the Word Window punch... scooting it over and punching again to make it long enough.
The shape of the tag is taken from the patterned paper.

Jess, this is very very cute. We don't have a container store up here where I live, but the next time I am Dallas, I will have to hit it up. I guess I could go to Michaels or elsewhere here and find one of those bags, but what an excuse to come to Dallas.
