First I stamped the image on black with versa mark ink and embossed it in white. I cut the card into thirds and trimmed away about 1/4 inch from the sides of each rectangle. Then I cut pink and backed all the rectangled. I punched a hole in the middle frame and tied a knot in pink ribbon and slipped it through the hole and fastened ends on the back. This rectangle only I raised with dimensionals. I glued the ribbon on the other two frames, wrapping the ribbon around the sides and leaving the other part of the ribbon to go over into the middle before I put on the middle frame. Simple card to make and one of my favorites.

That white embossing is AMAZING!! With the added ribbon it looks like real crochet.
For HYCCT I think bulk isn't as much of a problem as it is when we're mailing cards out ourselves ;-). I think quite often if patients or family pick them from the baskets, they're going to give them to other people without mailing - so I wouldn't worry at all about layers and ribbon.
