I don't think the tape is supposed to be centered on the roller. I think it's supposed to be lined up with the edge of the roller. But it certainly shouldn't be falling off. Did you fasten it back in with the red clamp-thingy before closing it up? Hope that helps!
she is right that it isn't supposed to be in the center. It is supposed to be on the edge. I would bet that you don't have the end of the roll fastened in right. Double check that. How long have you had it? Is this the first time you have changed the roll?
You may not have it wound tight enough or you may not be applying pressure evenly when you are using it. I have heard others with the same complaint only to find that they are tilting the gun slightly to see where the adhesive is going. The tape is meant to be on the edge of the roller so just use that as your guide and try not thinking about it too much. I usually go pretty fast with mine and don't worry much about where it is going. HTH
