A tall skinny gift box template for ya, it measures 1 1/8 inch x 5 inches.

There are two attachments here:

1) The altered template that you can print straight onto cardstock
2) The score guide with dotted lines for where to score

Deets:

This is originally from SU but I could not get their PDF to print fully & I didn't want all the score lines to show, so I resized it & removed the lines.
