Default Coupon holder template? Anyone have one of these?

I'd REALLY like to make a holder for coupons? Does anyone have a template/directions for this? Thanks.

This one from papertrey ink is really cute - you can download the tutorial here:

There is a great example on this site:

It says on papertrey ink that it is a recipe holder but on craft critique the lady used the template to make a really cute coupon holder.
