Acetate Gift Box Tutorial--help me find it, please

I saw this cute gift card set using SU! delicate doilies stamp set and the demo also made a box for the cards where the lid was made of acetate and she stamped the doilies on the box lid using stazon white ink.

Does anyone have a tutorial for how to make the box for standard A2 sized cards and their matching envelopes?
