Well, Ladies, the one on the card was purchased at Michaels...I found the packages on an end cap some time ago (6 in a package for just 25 cents! I just can't pass up a bargain!).
They are, however, very easy to make. The one on the card is just three loops of ribbon gathered together in the middle and secured with a few stitches. The "leaves" behind it are another loop of ribbon, twisted back on itself to form the points for the leaves. I just glued a small button in the center to finish it.
The ribbon this is made of is a little less than a 1/2" wide, but you could make them with any width. Check out Michaels and Walmart for ribbon flowers, if you don't want to attempt making them. They're usually in the sewing section.

Judy,
I think those are done by threading ribbon on a needle with a long eye and just looping it through the pierced holes. I thought I'd seen an example on this site, too. I'll do a search when I have more time.

Carol Duvall on HGTV.com has lots of guests making ribbon flowers, so you could go to that site and search for directions. They often use wire edged ribbon and tie the middles of flowers with the wire, I imagine we could do that also with regular ribbon and tie it with a piece of wire. I wish I was home to try this instead of working.

The snowflake brass template was used, but you can easily adapt that with the paper piercing template, or mark the same basic design on your CS and then punch.
Thanks so much for finding it!!! Now I have to figure out how to adapt it with our paper piercing template... or jus go and buy the snowflake brass template to do it!

Okay I'm lost- where does the brass template come in?
Jenn posted above about using the snowflake brass template as a guide for punching the holes into the cardstock, as was shown at regionals last year....
