This is cute. I have not done this, but here is how I would do it. Begin with a folded card. Decorate the front, eyeball about where the tag will be in the opened and closed position, punch out the hole in the card front with the word window punch. Then place the tag inside the folded card in the closed position and carefully place a pop dot in the word window slot on the right hand side. Make sure that the pop dot fits in the window slot and is only on the tag and not on the card. At this point, I would test to see if it will slide correctly. Apply the truck to the pop dot sandwiching it in the word window slot. Now for the inside. On a separate piece of paper, I would try to match the pattern that shows in the word window slot and adhere to the inside of the card. Finally, adhere the right and bottom edge of the folded card together. Does this made sense?
